You may order your DOL Card Replacement here. To qualify for a DOL Card Replacement, you must meet all the conditions below:
- You were enrolled in OSHA 10-Hour or 30-Hour training through 360training.com or its partners.
- You have called the 360training card activation line and verified receiving the original card.
- You have successfully passed the OSHA 10-Hour or OSHA 30-Hour Course within last 5 years.
- You have never requested an OSHA Replacement Card from 360training.com before.
Remember to log in to your student profile and verify the correct shipping address on your account. Make any corrections BEFORE you submit your request.
NOTE: Per Federal OSHA regulations, 360training is only allowed to issue one card replacement per cardholder and a card replacement will not be issued if an OSHA Outreach Training course was completed for more than five (5) years prior to the current date.
- The cardholder or the person who completed the course is the only person allowed to request a card replacement. It cannot be requested by an employer or 3rd party.
- Refunds are not allowed for extra replacement requested.

What Do I Do if My OSHA DOL Card is Lost, Damaged, or Stolen?
There are two ways to get an OSHA card replacement:
- Order the DOL Card Replacement Online here by clicking BUY NIOW (easiest and fastest way)
- Call Customer Support and submit your application over the phone
NOTE: When you order your replacement online, you may be required to log in to your student account. Before you provide payment information, remember to edit your student profile and ensure the correct shipping address is associated with your account.
When you call Customer Support, you’ll be asked to verify the following information:
- Full Name
- Completion Date
- Address on File
- Phone Number
You will also be asked about the training courses completed, such as whether the training was 10-hour or 30-hour. In addition, give us as much detail about how the replacement card was lost, damaged, or stolen, which will be useful in preventing identity theft.
Is Your Billing Address Different from Your Shipping Address?
Your billing address can be different. Please provide it when you enter your payment information because we will need to match it with the address on file with the credit card provider.
When you create your user profile, you can provide your shipping address, which should match the delivery address for where you want your DOL card shipped.
To ensure timely delivery of your DOL card, you or customer support can edit this at any time.
How Much Will the Student Be Charged for a Replacement?
OSHA DOL card replacement costs $89.00.
How Much Time Does It Take for a Student to Receive a Replacement Card?
360training offers three shipping methods:

When can I expect to get my OSHA DOL card?
Your official plastic, wallet-sized OSHA DOL card should arrive within approximately two (2) weeks following the completion of your course and survey. After the mandatory reporting process, the card will be printed and mailed via USPS First Class to the mailing address specified in your profile.
Upon successful course completion, you can access your training profile to either print or download the certificate of completion to your computer, smartphone, or tablet. While you await the arrival of your DOL card, you can use the certificate of completion as evidence that you've successfully finished the course.
Once you receive your OSHA DOL card, please verify receiving your card by phone within 24 hours of receipt of this letter.
- Call our automated line at 1-844-616-4690.
- Follow the prompt to type in the number on the card you received.
IMPORTANT: To ensure your card isn’t delayed, please verify that your mailing address is listed correctly in your profile.
I have not received my OSHA DOL card. What should I do?
It is very important to us that you receive your OSHA DOL card promptly. Upon completion of the course, you will receive a temporary certificate of completion from OSHA.com. In 2 weeks, your DOL wallet card will arrive in the mail.
Please allow sufficient time for your card to reach you through the mail. The end of course survey must be submitted within 3 months of course completion or a wallet card will not be issued.
In the event that you do not receive it within 2 weeks of completing your course, please contact our customer support team at (877) 881-2235. Wallet cards that can not be delivered by the USPS will be returned to our fulfillment center. If your card is returned, we will work with you to resend the card to the proper address. It is necessary to allow sufficient time for the card to be returned. If it is not returned by the USPS, the request will be processed as a replacement card and will be subject the associated purchase and delivery fees.
