OSHA DOL Card Upgrade

Before you purchase this upgrade, please verify you meet all the following conditions:

• You have previously received a paper DOL card through 360training or one of its partners.
• You are requesting this upgrade for yourself only. DOL cards are only issued to individual learners, not their employers.
• You have possession of your existing paper card and can email an image of it to support@360training.com.
• You agree to promptly send your paper card to 360training via a return envelope that will be provided to you.

NOTE: Per OSHA regulations, you’re only allowed one replacement card in case of loss or damage. This upgrade does not count as a replacement.

What to expect

• Once you are sure you meet all the conditions listed above, click BUY NOW to add this item to your cart.
• Verify during checkout that your name and address are accurate.
• After the purchase is complete, email a clear image (digital photos, or scanned images are acceptable) to support@360training.com.
• Our customer support team will process you request and send you a return postage paid envelope.
• Place your paper card into the return envelope and mail it to 360training promptly.
  (Failure to return the paper card within 10 days may result in your plastic and paper cards being revoked and reported to OSHA.)
• You will receive your durable plastic card within 2 weeks.

NOTE: Before you provide payment information, please remember to edit your student profile, and ensure the correct shipping address is associated with your account.

Is your billing address different from your shipping address?

Your billing address can be different. Please provide it when you enter your payment information because we will need to match it with the address on file with the credit card provider.

When you create your user profile, you can provide your shipping address, which should match the delivery address for where you want your DOL card shipped.

To ensure timely delivery of your DOL card, you or customer support can edit this at any time.

How much time will it take for a student to receive a replacement card?
