Can I change the name on my online completion certificate as well as on my DOL Card after completing my OSHA 10- & 30-hour Outreach Training courses?

Unfortunately, we do not accommodate any full name change requests for any completed OSHA 10/30 Hour Outreach Training courses.

Upon taking your course, you have acknowledged that the name you have provided on your account is correct and matches your Government Issued ID. This is where we based in printing the name on your OSHA DOL Card. See the sample snapshot below of the acknowledgment page that you have acknowledged:
